A person wanting to create either an enduring or general power of attorney, or an enduring power of guardianship needs to complete a specific form while she or he has legal capacity. It is recommended that forms be completed so as to be available should any unforeseen circumstances arise.

Likewise, enduring power of guardianship forms are in the Enduring Power of Guardianship Kit.

The kits are available from any office of the Legal Services Commission, Service SA and the Office of the Public Advocate.
